Our services span a wide range of social, medical, and educational support to help you navigate life's complexities. Home Again Program
With COHYA, you are not alone in your transition. Our Home Again Services, in collaboration with SCDHHS, provide essential support to Medicaid beneficiaries who have resided in nursing facilities or hospitals for over 60 days who are prepared to return to the community. We provide the guidance, resources, and encouragement needed to help you successfully transiton home.
